To receive a complete Application Packet to be filled out, please send an email to the IFOV Chair Dr. Van Wilson with your complete mailing address and one will be sent.

If you are planning on applying through the Health Science Center, you can get all the information from their admissions web page.

Once you have the Application Packet completed (this is for both TAMU and Health Science Center), please send the application, letters of recommendation and application fee to the following address:

      Once received, you will receive a letter stating that we have
      received the packet. The application will then be processed and
      sent through the correct channels. Once a decision has been made
      you will be contacted.

**NOTE: If the application packet is not sent to the above address, it may not reach the intended person(s).
